    The Permanency Specialist provides specialized administrative support and case coordination to assist in achieving permanency. Overall, the Permanency Specialist contributes to the CHS high performance culture by exhibiting our values and providing quality results that keep CHS as the premier provider of child, family and behavioral health support services.  Our end goal?     Responsibilities

    YOU WILL:

    • Provide specialized administrative support and case coordination to assist in achieving permanency.
    • Prepare and process necessary file documentation in required systems.
    • Prepare documentation in preparation for permanency staffings and court proceedings.
    • Facilitate the agenda, tracking attendance, documenting the outcome and follow-up from permanency meeting.
    • Track judicial requirements and timeframes.
    • Verify compliance of permanency staffings and closure recommendations.
    • Coordinate transfer of case files for finalizations.
    • Coordinate permanency plans and visitation agreements in cases where siblings are separated.
    • Coordinate staff and track all cases for permanency.
    • Assess the appropriateness of individual case goals.
    • Refer cases to and participate in permanency round table staffings.
    • Track and coordinate Family Finders referrals.
    • Maintain tracking systems.
    • Document all staffings in FSFN statewide database.
    • Create adoption information page when child is free for adoption.
    • Register new adoption clients in the Adoption Exchange System (AES), CHS website, and refer for the Heart Gallery.
    • Assist with the completion of adoptions packets in preparation for case transfer from reunification to adoptions unit.
    • Contribute to district staffing meetings by participating, coordinating, and documenting staffings as needed.
    • Participate in adoption sibling separation staffings.
    • Facilitate reunification staffings.
    • Coordinate all case plan conferences and document in FSFN the attendance and outcome of the conference.
    • Maintain regular communication with case managers, supervisors, and program managers.

    Qualifications

    YOU HAVE:

    • Perform under strong demands in fast-paced, diverse environment
    • Handle confidential information appropriately
    • Perform at a high level of autonomy, with minimal supervision
    • Work on multiple tasks while maintaining a high attention to detail, accuracy and quality
    • Commit to effectively and efficiently providing high levels of customer satisfaction with positive service delivery results
    • Demonstrate the behaviors of the CHS Common Bond Values
    • Interact effectively with children/families from diverse backgrounds
    • Set appropriate limits and boundaries with clients; act decisively to protect clients, when necessary
    • Assess and apply proper intervention strategies
    • Identify and understand environmental stressors

     

    YOU NEED:

    • Bachelor’s degree in a human services field
    • Certified Child Protection (CPP) Certification
    • Two years of experience delivering services in a dependency environment
