    As a Case Manager II you will serve as a Neonatal Outreach Specialist for the Healthy Families Coalition contract to ensure that infants in the Neonatal Intensive Care Unit at the local hospitals have a pediatric medical provider, attempting to engage mothers in the Healthy Start program, encouraging and providing breastfeeding support to mothers, and providing referral and assessment services to referred families.  The NOS will work in conjunction with the Women’s Intervention Specialist and may assist in conducting biopsychosocial assessments.

    • Act as a liaison between the hospital NICU, Healthy Start, CMS, and/or the infant’s medical provider
    • Verify that a Healthy Start Infant Screen has been completed on the NICU infant and research the Well Family System to see if the MOB is currently receiving Healthy Start services
    • Attempt to contact and engage MOB in Healthy Start by completing an Initial Contact and having MOB consent to participate.
    • Provide outreach and linkages to ensure the NICU infant has an appointment for follow up with a pediatric medical provider and/or a referral to CMS within 5 days of hospital discharge.
    • Encourage and provide breastfeeding support, education, and/or referral to the MOB if medically appropriate, and MOB is interested.
    • Complete a Healthy Start Risk Assessment on the MOB when possible and refer according to risks/needs identified.
    • Work in conjunction with the Women’s Intervention Specialist (WIS) and may provide coverage for the WIS as needed to include conducting BPSAs.



    • Bachelor’s degree in Social Work, Human Services, Psychology, or related field 
    • Minimum of two years experience providing assessment and referral services to adults and/or adolescents suffering from substance related disorders
